If you are having issues, instead you can email the details on the nomination form to NEAwards@health.gov.au along with your Statements of Achievement and other supporting documentation (as outlined in the guidelines). If you have any further queries regarding this please contact free call number 1800 641 538.

The Nomination Form is able to be edited online by simply clicking in the boxes where you want to type your information. Please make sure you print the form off and save all your changes.

Once you have completed your form, you may lodge it electronically by emailing it to NEAwards@health.gov.au (make sure you attach your supporting documents to the email) or by printing the form and attachments and sending to:

Completing Your Nomination

Ensure that in completing your nomination form you have:

Selected one of the award categories
Completed the nominee details
Completed and attached the three Statements of Achievement (up to 500 words for each statement)
Completed the Nominator details
Completed the Referee details
Attached a Letter of Support
Checked that the Agreement to Nomination is signed (or that you have acknowledged the email disclaimer)

General Information

Statements of Achievement

All nominees must address the three Statements of Achievement and all nominations will be assessed against the following criteria.

Sustainability
2. How was it achieved? Innovation
Adaptability
3. Why is the achievement worthy of recognition? Cultural Relevance
Leadership

Effectiveness: The extent to which positive and measurable results are being achieved

Sustainability: The extent to which achievements can be maintained.

Innovation: The extent to which new concepts, systems and relationships are developed and adopted by the community and/or organisation.

Adaptability: The extent to which achievements can be shared and used by others involved in Aboriginal and Torres Strait Islander health.

Cultural Relevance: The extent to which achievements respect and reflect Aboriginal and Torres Strait Islander culture.

Leadership: The extent to which a vision is being communicated to and embraced by the community and/or organisation.



Individual Category

Nominations must include a letter of support (up to 500 words) from an Aboriginal and/or Torres Strait Islander person of standing within the community where the individual works. The letter must demonstrate that the person being nominated is recognised by the community for their work in the health sector.

Organisation Category

Nominations must include a letter of support (up to 500 words) from a stakeholder. This letter must demonstrate that the organisation is contributing to positive health outcomes for the community.

Finalists

All finalists will be visited by members of the judging panel. The aim of the site visits is to verify the information provided in the finalists' nomination forms. The judges will have an opportunity to see the finalists and their work in the community.

The site visits will also provide finalists with an opportunity to showcase their achievements.

Site visits will involve meetings with members of the community and consultations with the organisations and stakeholders.

Finalists may also be invited to attend relevant conferences and workshops in the Awards year or subsequent years.
